Our Home Care program can help you manage your government funded Home Care Package. Following your assessment to be eligible for a home care package by the Aged Care Assessment Service, the Government will allocate a package to you, when one becomes available.

Shepparton Villages Case Manager can explain to you how Home Care Packages can work for you including fees and services.

If you wish to take up your package with Shepparton Villages we will work with you to develop your care plan and what services you require to support your plan. Once you are happy with your plan we will coordinate the services for you and manage your package for you.

Ongoing contact is made through phone calls and home visits. We want to help keep you safe and remain at home happy and well.

Short Term Restorative Care (STRC)

Short Term Restorative Care is an eight-week program designed to reverse or slow the functional decline in an older person, and to optimise their independence in their own home.

It is delivered in the client’s home with a multi-disciplinary approach that includes the client’s GP, and a minimum of two other disciplines such as physiotherapy or occupational therapy.

Shepparton Villages is an approved provider of Short Term Restorative Care and can deliver packages across the Hume Region.

Christine Symmons, our Community Services Program Manager will work with you to set your goals and ensure the maximum benefit of your program is achieved.
